β’ Constitution-and By-Laws approved by the Officers and the Advisers
β’ Updated list of members
β’ Bio data of the Organization Officers
β’ Certificate of Good Moral of the Officers
β’ Acceptance Letter from the Adviser

β’ Accomplishment Report
β’ Financial Report
β’ Previous Plan of Activities
β’ Certificate of Audit
